Integratedliving is a place where you can shine — where your ideas are valued, and your passion makes a real difference. We’re committed to empowering our team and doing our best every day to help our clients live better lives than yesterday. As our organisation continues to expand rapidly, we’re looking for passionate Case Managers to join our team and be part of this exciting journey.

About the Role

As a Case Manager, you’ll support clients across Hervey to Sunshine Coast with care that’s truly personal. You’ll work closely with them, their families and healthcare professionals to create tailored plans that fit their lives, goals, and routines.

It’s all about helping people stay independent and feel their best—whether it’s something big or one of those little wins that mean everything.

We’re looking for someone who is based between Hervey Bay and Sunshine or in between, passionate about health and wellbeing, excels at building relationships, and thrives both independently and as part of a team.

To be successful, you’ll have:

  • Certificate III or higher in Case Management, Aged Care, Disability, Community Services, or a related field
  • Previous experience in case management within aged care, disability, or youth services
  • Good understanding of Home Care Packages and the new Support at Home program
  • Excellent communication and interpersonal skills
  • A valid driver’s licence and access to a reliable vehicle
  • Confidence to work autonomously while collaborating with internal and external stakeholders

What We Offer

  • Competitive Salary: $78,091.92 + 12% super + up to $18,550 tax-free salary packaging
  • Perkbox: Get access to 9,000+ discounts and save $ on your groceries, online shopping, travel, fitness, and more.
  • Work-Life Balance: Regular, predictable hours in a hybrid working environment
  • Growth Opportunities: Ongoing learning and development
  • Impactful Work: A career where your work truly makes a difference
  • Supportive Culture: Inclusive, respectful, and focused on your well-being

If you're ready to take the next step in your career and want to be part of a compassionate team making real change in communities, we’d love to hear from you!
