Role Responsibilities:
-
Health education & promotion – support clients and carers with tailored health advice.
-
Deliver wellness programs – including the Wellness for Independence® programs both in-person and online.

Use Telehealth – to monitor clients and enhance health outcomes remotely.

Provide direct clinical care – across multiple programs, including wound, diabetes, medication, stoma, and catheter management.

Collaborative, client-centered care – develop and evaluate nursing care plans in partnership with clients and Allied Health teams.
What you need to be considered for the role?
-
Knowledge of community nursing & primary health care – including experience within a consumer-directed care model.

Minimum 1 year of relevant experience – in community, acute, or aged care settings as an RN.

Ability to work independently and collaboratively – demonstrating flexibility and reliability.
-
Strong organisational and tech skills – including use of electronic devices and client management systems.
-
AHPRA registration & advanced clinical care experience – covering areas such as chronic disease, palliative care, medication, wound/stoma/catheter care, and diabetes management.
