About the Role

As a Case Manager, you’ll support clients across Illawarra with care that’s truly personal. You’ll work closely with them, their families and healthcare professionals to create tailored plans that fit their lives, goals, and routines.

It’s all about helping people stay independent and feel their best—whether it’s something big or one of those little wins that mean everything.

We’re looking for someone who is passionate about health and wellbeing, excels at building relationships, and thrives both independently and as part of a team.

To be successful, you’ll have:

  • Certificate III or higher in Case Management, Aged Care, Disability, Community Services, or a related field
  • Previous experience in case management within aged care, disability, or youth services
  • Good understanding of Home Care Packages and the new Support at Home program 
  • Excellent communication and interpersonal skills
  • A valid driver’s licence and access to a reliable vehicle
  • Confidence to work autonomously while collaborating with internal and external stakeholders
