Whether you're based in Far North Queensland, Townsville, Hervey Bay, Maryborough, the Hunter Region, Illawarra, ACT, Gippsland, or even Hobart — this opportunity could be just right for you.
Your Day-to-Day Might Include:
- Visiting clients at home or virtually to co-design personalised care plans that focus on reablement and wellness.
- Running engaging group sessions that promote healthy living and self-management.
- Collaborating with GPs, specialists, and other health professionals to deliver truly holistic care.
- Using digital tools to provide virtual coaching and support.
- Building strong local networks and promoting at Home Short Term Pathway – Restorative Care in your community.
What you’ll bring:
- A degree in Nursing or Allied Health, with current AHPRA or relevant registration.
- At least 3 years’ experience in clinical care, health coaching, or care coordination—especially in aged or community care.
- A passion for health promotion, preventative care, and client empowerment.
- To confidently navigate digital platforms for virtual care delivery, accurate documentations and communication a high-level computer skills is essential
- A valid driver’s licence and a love for getting out into the community.
