We’re looking for a Permanent Full Time Community Care Partner to join our NSW Far North Coast Community Care Team!
This is a rewarding opportunity to partner with older clients to coordinate Support at Home services.

As a Community Care Partner with integratedliving, you will:

• Coordinate and monitor in-home services with our care and health teams
• Manage Support at Home budgets effectively
• Advocate for clients’ rights and wellbeing
• Maintain accurate documentation and reporting
• Build connections with community organisations to enhance support options

What you’ll bring:

• Certificate III (or higher) in Aged Care, Disability or Equivalent
• Experience in case management or aged care service delivery
• Excellent communication, organisation, and problem-solving skills
• A kind, empathetic approach and commitment to person-centred care
• A valid driver’s licence and current police check
