We’re looking for a compassionate Community Registered Nurse to join our Gippsland team, supporting clients across Morwell/Traralgon and surrounding areas to live well and independently at home.
Part-time | 3 days per week | No Night Shifts
Why this role matters
In regional and remote areas, access to timely, person-centred care is critical. As a Community RN with integratedliving, you’ll build trusted relationships with older clients and their families, delivering clinical care in the home and coordinating services that help people stay safe, comfortable, and connected.
What you’ll do
- 
Provide in-home clinical nursing (e.g., wound care, medication management, catheter care, chronic disease support).
 - 
Complete holistic assessments, develop care plans, and review outcomes with a strong reablement focus.
 - 
Educate clients and carers on self-management and health literacy to support independence.
 - 
Coordinate with GPs, AHPs, hospitals and internal teams to ensure seamless, person-centred care.
 - 
Travel between client homes across Morwell, Traralgon and surrounds (mileage reimbursed).
 
About you
- 
AHPRA-registered RN with minimum 1 year experience in community, primary health, aged care or similar.
 - 
Confident delivering clinical care autonomously in home and community settings.
 - 
Strong organiser with sound clinical judgement and a solutions mindset.
 - 
Police check, Working with Children check, CPR, Valid driver's licence and access to a reliable, insured vehicle
 - 
(Desirable) Experience in wound care, chronic disease management, palliative care, or hospital-in-the-home.
