Join integratedliving as a Senior Health Coach and empower older Australians to live independently through personalised coaching in our 8-week Short-Term Restorative Care Program. This flexible, community-based role focuses on care coordination—not direct care—working closely with clients, families, and health professionals to support meaningful, lasting outcomes.

Key Responsibilities –

Conduct in-home client visits to assess, plan, and coordinate 8-week Short-Term Restorative Care (STRC) programs.

Develop personalised care plans and provide ongoing support to promote independence and self-care.

Educate clients and families on wellness, reablement, and chronic disease management.

Facilitate group sessions focused on healthy lifestyles and self-management.

Coordinate with GPs, specialists, and Allied Health for a holistic care approach.

Promote the program locally, build referral networks, and maintain strong relationships with health professionals.
What You’ll Need
-
Tertiary qualification in Nursing or Allied Health, with AHPRA or relevant professional registration.

Minimum 3 years' clinical or health management experience, ideally within aged care or community health settings.

Strong understanding of primary health care, community aged care guidelines, health promotion, and evidence-based practice.

Proven ability to conduct case management, health assessments, and deliver virtual care using digital platforms.

Excellent interpersonal, organisational, problem-solving, and time management skills with the ability to work both independently and in a team.

Commitment to ongoing professional development, with a valid driver’s licence and willingness to travel within the region.
