About the Role
- Position Type: Permanent Part-Time
- Schedule: Thursdays & Fridays | 7:00 AM – 3:30 PM
- Location: Ballarat – serve your local community
- Travel: Required to deliver in-home clinical services
As a Community Registered Nurse, you’ll play a vital role in empowering clients to live safely and independently. You’ll provide high-quality clinical care and build trusted relationships with clients, carers, and health professionals.
Your Responsibilities Will Include:

Delivering clinical care under Home Care Package (HCP), CHSP, and other aged care programs

Managing wound care, medications, chronic disease, catheter/stoma care, and palliative support

Collaborating with Allied Health teams for holistic, goal-based care planning

Providing education and health coaching via Telehealth and face-to-face visits

Supporting group wellness programs including our award-winning Wellness for Independence®
What You’ll Need to Succeed

AHPRA registration as a Division 1 Registered Nurse

Minimum 1 year of experience in aged care, community, or acute settings

Current Police Check, Working with Children Check, CPR, and Driver's Licence

Ability to work independently and manage a dynamic client schedule

Sound knowledge of community nursing and person-centred care

Comfortable using digital tools and client management systems

Advanced clinical skills across medication, wound, chronic disease, and palliative care
