As our organisation continues to expand rapidly, we’re looking for passionate Care Partners to join our Care Partnering team and support clients across Devonport and surrounding communities.
About the Role
As a Care Partner, you’ll support older Australians in Devonport and nearby regional communities to live safely, independently and well in their chosen home. You’ll build trusted partnerships with clients, families, carers and local health professionals, delivering care that’s truly personal and locally responsive.
You’ll act as the primary point of contact for your caseload, coordinating services across Support at Home pathways and ensuring each client experiences a seamless, connected journey. From proactive care management to early risk identification, you’ll play a central role in improving wellbeing, independence and quality of life.
This role is ideal for someone who is passionate about health and wellbeing, excels at building relationships, and thrives both autonomously and within a supportive multidisciplinary team.
What You’ll Be Doing
- Client partnership — building strong, ongoing relationships to understand goals, preferences and evolving needs
- Holistic assessments — considering clinical, functional, social and emotional factors
- Care planning — developing personalised, outcome‑focused plans that support independence and reablement
- Service coordination — integrating services across Support at Home, assistive technology, home modifications and end‑of‑life pathways
- Budget management — supporting clients to understand their funding and managing budgets responsibly
- Risk identification — proactively reviewing care plans and addressing emerging risks
- Community connection — strengthening local relationships and promoting integratedliving services
- Quality and compliance — ensuring safe, compliant care aligned with Aged Care Quality Standards
What You’ll Bring
- Certificate IV or higher in Aged Care, Disability, ,Nursing, Community Services, or a related field
- Experience in care planning, assessment or service coordination within aged care, disability, community or youth services
- Understanding of Support at Home funding and programs
- Strong communication and relationship‑building skills
- Confidence working autonomously while collaborating with internal and external stakeholders
- A valid driver’s licence and reliable, comprehensively insured vehicle
