We’re looking for an experienced Care Partner to support older Australians across Launceston and surrounding Northern Tasmanian communities, helping them live independently, safely and comfortably in their own homes.

At the heart of this role are your clients.

You’ll build genuine relationships, understand what matters to each person and coordinate the services they need to live well at home.

You’ll also be responsible for bringing new clients on board — with a target of around two new clients per week — completing the associated paperwork and maintaining regular contact with every client through monthly monitoring calls.

Because great client outcomes and commercial sustainability go hand in hand.

What you’ll be doing

You’ll be responsible for:

  • Building strong, genuine relationships with clients and their families
  • Understanding client needs, goals and preferences
  • Developing and coordinating personalised care plans
  • Coordinating services and providers to ensure clients receive the right support
  • Advocating for clients and making sure their voice is heard
  • Managing Home Care Package budgets responsibly
  • Bringing new clients into the service and completing onboarding documentation
  • Completing monthly monitoring calls
  • Identifying changes in client needs and coordinating appropriate responses
  • Staying on top of documentation, administration and KPIs
  • Working collaboratively with your manager and wider team

This role is about care coordination, relationships and client outcomes — not providing hands‑on nursing care.

Who we’re looking for

You might be:

  • A Support Worker ready to take the next step
  • A Care Coordinator or Case Manager wanting more autonomy
  • An Aged Care professional who understands what great support looks like
  • A Case Manager from another provider who knows how to manage clients, budgets and services
  • A Nurse looking to move into care coordination (non‑clinical role)

A Certificate IV or higher in aged/community care, care coordination or equivalent experience is required, along with experience in case management, wellness/reablement or aged care service delivery.
