We’re seeking an accredited Social Worker to deliver high-quality, evidence-based services across the community and via flexible work-from-home arrangements.
At integratedliving, our Allied Health Services provide direct and virtual services to promote and improve client health and wellbeing outcomes, consistent with the integratedliving signature experience.
What You’ll Do:
As part of a multidisciplinary team, you will:
- Conduct comprehensive assessments and develop evidence-based care plans tailored to each client’s needs
- Provide direct and virtual Social Work services, including interventions, therapy, and support for clients and their carers
- Facilitate access to Telehealth and other specialised services or equipment in clients’ homes
- Collaborate with case managers, health partners, and community stakeholders to ensure holistic care
- Deliver health and wellbeing education through programs, workshops, and community forums
- Participate in case conferences, review client progress, and adapt care plans as needed
- Support continuous improvement, compliance, and quality in all aspects of service delivery

What We’re Looking For:
- Bachelor’s degree or higher in Social Work
- Accredited Social Worker with AASW membership and accreditation
- Experience in community services or comparable sectors, ideally within multidisciplinary teams
- Strong communication and interpersonal skills, with the ability to engage a variety of stakeholders
- Well-developed IT skills and experience using client management software
- Commitment to continuous professional development and best-practice Social Work
- Satisfactory Police Check & WWCC
- Current Driver’s Licence
- Current First Aid & CPR
