  • Location: Serving Ballarat and surrounding areas
  • Travel: Must be willing to travel for client care

Week 1 Mon 0700 -1430, Tuesday 0700 - 1430, (14hrs)
Week 2 Mon 0700 -1430, Tuesday 0700 - 1430 Sunday 0700- 1100 (18hrs)
Week 3 Mon 0700 - 1430, Tuesday 0700 - 1430 (14hrs)
Week 4 Mon 0700- 1430, Tuesday 0700-1430, Saturday 0700-1100 (18hrs)

Role Responsibilities & Qualifications

  • Provide health promotion and education to clients and their carers
  • Promote and deliver our award-winning Wellness for Independence® programs, both in-person and virtually
  • Monitor and improve health outcomes through Telehealth.
  • Direct Care Services: Provide care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.
  • Centered Care: Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence with support from Allied Health clinicians.

What you need to be considered for the role?

  • Understanding of community nursing and primary health care principles, including working within a consumer-directed care framework

  • At least 2 years of experience in community, acute, hospital or aged care

  • Experience working independently or as part of a team

  • Strong organizational skills and attention to detail

  • Proficiency with electronic devices and client management systems (smartphones, tablets, laptops, email, etc.)

  • Current Registration with AHPRA, Police check, working with children check and CPR

  • Experience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management
